Job Summary
Coordinate the RAI process, ensuring timeliness and completeness of MDS, CAAS, and Interdisciplinary Care Plans while assisting the Director of Nursing with federal, state, and certification guidelines. Initiate and monitor RAI tracking, discharge/re-entry, and Medicaid forms through the AHT system, collecting data and interviewing staff to support accurate assessment reference dates and diagnosis coding. Facilitate interdisciplinary scheduling, obtain required signatures, and submit resident assessment data to state and federal agencies, responding to audit findings and correcting deficiencies. Maintain confidentiality, stay current on industry changes, and assign or instruct staff on RAI processes and clinical computer systems.
Required Qualifications
- Must have a current LVN license
- Excellent knowledge of RAI Process, the federal Medicare PPS process, and Medicaid reimbursement as required
- Thorough understanding of the Quality Indicator process
- Knowledge of the OBRA regulations and Minimum Data Set
- Knowledge of the care plan process
- Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching and lifting up to 50 pounds with or without reasonable accommodations
Desired Qualifications
- Minimum of two (2) years nursing experience in skilled nursing facility preferred
