Job Summary

Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in compliance with Medicare, Medicaid, OBRA, and CMS regulations. Ensure assessments reflect the physical, mental, and psychosocial status of each resident while maintaining accurate documentation for service reporting and reimbursement. Participate in pre-admission processes, case management, and interdisciplinary team meetings to optimize MDS/Case Mix and facilitate efficient care plan development. Conduct regular audits of the MDS process, validate coding documentation, and track default days to prevent further action. Perform concurrent MDS reviews, manage Modification/Inactivation of assessments, and ensure timely electronic submission of all Minimum Data Sets. Communicate with Regional Care Management Specialists, Business Office, and external case managers regarding eligibility, benefit periods, and utilization reviews. Assist in the orientation and training of new associates on RAI processes and federal/state regulations. Maintain current knowledge of reimbursement regulations and utilize AIS training modules for competency development.

Required Qualifications

  • Must be a graduate of an accredited school of nursing with current R.N. or L.V.N.
  • Licensure in the state in which employment occurs
  • Minimum of two years health care experience
  • Must possess superior clinical assessment and documentation skills
  • Must demonstrate strong interpersonal skills and ability to work well in a team environment
  • Ability to work flexible work hours to support business requirements
  • Ability to utilize both local and corporate resources in the execution of job responsibilities
  • Must possess excellent verbal and written communication and presentation skills
  • Competency with standard office software applications as well as software applications related to MDS/RAI processes
  • High initiative and ability to efficiently and effectively lead interdisciplinary teams and coordinate and manage RAI process
  • Must be able to lift 1-25 lbs
