MDS Nurse (Licensed Practical Nurse/LPN)

MDS Nurse responsibilities include completing and submitting accurate MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations; review and report changes in a patient’s condition identified by MDS; provide education to direct care staff on CMS RAI Manual updates; assist with interdisciplinary care planning, final validation reports, and billing readiness; participate in PPS and other required meetings; perform as a staff nurse when needed; uphold patient rights, safety, confidentiality, and professional conduct; maintain attendance, teamwork, and effective communication; physically assist patients (sit/stand/move) and maintain English language proficiency; Life Care equal opportunity employer.
