Job Summary
MDS Nurse (LPN) responsibilities include completing and submitting accurate MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations; educating direct care staff on CMS RAI updates; reviewing Interdisciplinary Comprehensive Care Plans; ensuring CMS QIES documentation is accepted prior to billing; monitoring assessment timeliness; attending PPS/Triple Check meetings; performing staff-nurse duties as needed; maintaining confidentiality and professional conduct; evacuating patients if needed; and collaborating with the interdisciplinary team to uphold quality patient care in a post-acute care setting.
Required Qualifications
- Licensed Practical Nurse (LPN) license in good standing in applicable state
- Two (2) years nursing experience with geriatric nursing preferred
- CPR certification (on hire or during orientation) and current CPR
- Education from accredited LPN program
- Ability to interpret CMS RAI Manual Regulations and Life Care standards
- Ability to implement and interpret department policies and procedures
- Ability to communicate effectively and work on an interdisciplinary team
- Proficient in English
- Demonstrated integrity and professionalism
