Job Summary
Collaborate within the interdisciplinary team to develop individualized care plans and monitor documentation for accuracy and compliance. Support quality improvement initiatives while helping maximize reimbursement and promoting high-quality resident care. Maintain strong knowledge of long-term care regulations and assessment processes to ensure accurate data entry. Work Monday through Friday in a skilled nursing facility focused on dignifying long-term care and improving daily resident lives.
