Job Summary
Coordinate the Minimum Data Set (MDS) process for residents in accordance with regulatory guidelines, conducting comprehensive assessments of health status and care needs. Collaborate with interdisciplinary teams to develop individualized care plans and ensure accuracy and completeness of MDS assessments and documentation. Stay updated on changes in regulations and requirements, then train and educate staff on MDS processes and documentation standards. Participate in quality improvement initiatives and audits while communicating effectively with residents, families, and healthcare professionals. Requires current licensure as an RN or LPN, with previous experience as an MDS Coordinator preferred.
Required Qualifications
- Current licensure as a Registered Nurse (RN) or LPN
- Knowledge of MDS process and regulations
- Strong attention to detail and organizational skills
- Excellent communication and interpersonal abilities
- Ability to work collaboratively in a team environment
Desired Qualifications
- Previous experience as an MDS Coordinator preferred
- Proficiency in electronic health record systems preferred
