MDS Coordinator

MDS Coordinator responsible for developing, monitoring, auditing, and modifying each resident’s care plan to meet individual needs and goals; perform resident assessments and assist in discharge; ensure residents receive a high standard of care in a skilled nursing setting. Requires an unencumbered State of Iowa RN License or LPN certificate, graduation from an accredited nursing program, and CPR Certification. Prior MDS Coordinator experience and RN experience in a Skilled Nursing Facility are preferred. Will include responsibilities related to knowledge, skills, and attention to detail to ensure resident comfort and safety.
