MDS Coordinator

Assess residents' behavioral, psychological, clinical, nutritional, and physical needs to create individualized care plans. Collaborate with nursing, physical therapy, and nutrition teams to ensure care plan guidelines are followed and compliant with ethical standards, Medicare requirements, and Federal and State regulations. Oversee and facilitate the completion of resident assessments while maintaining compliance with all applicable regulations. This role requires an RN or LVN license, Skilled Nursing Facility experience, and the ability to pass a background check. Full-time position in Pasadena, TX.
