MDS Coordinator

Job Summary
Oversees the MDS process and coordinates completion of the MDS, CAA’s, and care plans within regulated time frames; schedules and coordinates the RAI process with the interdisciplinary team; conducts resident assessments through physical examination, interviews, and chart review; communicates resident care needs with care givers and physicians across nursing, social services, therapy, dietary, and activity staff; reviews information from hospital and outside sources to inform care planning; initiates and coordinates significant changes to MDS and PPS processes; conducts PPS meetings and maintains the PPS board to monitor Medicare days and RUG utilization; stays current with AANAC requirements.
Required Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
- AANC certification a plus
- RAC-CT
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred
- Experience as an MDS Nurse or acceptable exemption required
