Job Summary
Assure completion and accuracy of the Resident Assessment Instrument (RAI) process from MDS through care plan implementation. Monitor RAI tracking, discharge/reentry, and data collection for each resident while interviewing staff to verify MDS 3.0 standards. Facilitate interdisciplinary scheduling for assessments and care plans per OBRA and PPS guidelines, ensuring accurate determination of the Assessment Reference Date and obtaining required signatures. Coordinate resident care planning conferences, communicate outcomes to staff and residents, and assist the Director of Nursing with admission care plan initiation. Correct final MDS submissions to State and Federal agencies, maintain validation records, and assign or instruct staff on RAI, PPS, and clinical computer systems. Participate in daily stand-ups to address PPS timeline changes and deficiencies.
Required Qualifications
- Currently licensed as an LPN or RN in this state
- CPR certified
- At least 1 year of experience in MDS
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of the Federal Medicare PPS process, and Medicaid reimbursement
- Knowledge of the OBRA regulations and Minimum Data Set
- Knowledge of the care planning process
- Demonstrated leadership ability
- Excellent communication (oral and written) and public relations skills
- Excellent organizational skills with an acute attention to detail
- Computer literate
- Florida AHCA Clearinghouse background screening
