MDS Coordinator

Job Summary
Assure the completion and accuracy of the Resident Assessment Instrument (RAI) process from the MDS through the implementation of the care plan. Monitor RAI tracking, discharge/reentry, and data collection while interviewing staff to verify compliance with Federal, State, and Certification guidelines. Facilitate interdisciplinary scheduling for assessments and care plans, obtain required signatures, and assist the Director of Nursing with identifying significant changes and physician orders. Coordinate resident care planning conferences, communicate outcomes to staff and residents, and submit final MDS data to State and Federal agencies. Act as a resource for computer issues related to the MDS process and assign instruction on RAI, PPS, and clinical systems to all staff.
Required Qualifications
- Currently licensed as an LPN or RN in this state
- CPR certified
- At least 1 year of experience in MDS
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of the Federal Medicare PPS process, and Medicaid reimbursement
- Knowledge of the OBRA regulations and Minimum Data Set
- Knowledge of the care planning process
- Demonstrated leadership ability
- Excellent communication (oral and written) and public relations skills
- Excellent organizational skills with an acute attention to detail
- Computer literate
- Florida AHCA Clearinghouse background screening
