MDS Coordinator

Job Summary
Coordinate the entire Resident Assessment Instrument (RAI) process, ensuring accuracy, timeliness, and completeness of MDS, CAAs, and Interdisciplinary Care Plans. Assure compliance with Federal, State, and Certification guidelines by initiating RAI tracking, collecting resident data, interviewing staff, and facilitating assessment reference dates. Monitor discharge/reentry, obtain necessary signatures, and assist the Director of Nursing with identifying significant changes and physician orders. Participate in daily stand-ups to communicate PPS timelines, deficiencies, and care plan needs while acting as a resource for computer issues. Submit final MDS assessments to State and Federal agencies, maintain validation records, and assign or instruct staff on the RAI process, Medicare PPS, Medicaid, and clinical systems.
Required Qualifications
- Currently licensed as an LPN or RN in this state
- CPR certified
- At least 1 year of experience in MDS
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of the Federal Medicare PPS process, and Medicaid reimbursement
- Knowledge of the OBRA regulations and Minimum Data Set
- Knowledge of the care planning process
- Demonstrated leadership ability
- Excellent communication (oral and written) and public relations skills
- Excellent organizational skills with an acute attention to detail
- Computer literate
- Florida AHCA Clearinghouse background screening
