MDS Coordinator

Assure completion and accuracy of the RAI process from the MDS through implementation of the care plan, initiating and monitoring tracking for discharge and reentry. Collect resident data and interview staff to verify standards of practice, facilitating accurate determination of the Assessment Reference Date. Provide interdisciplinary schedules for all MDS assessments and care plans, ensuring appropriate signatures and that care delivered is supported by documentation. Assist the Director of Nursing with identifying significant changes, physician orders, and verbal reports to reflect updates in the MDS and care plan. Coordinate scheduling for resident care planning conferences and communicate outcomes to staff, residents, and responsible parties. Correct and submit final MDS data to State and Federal agencies, maintaining validation records and ensuring timely submission. Assign, assist, and instruct all staff in the RAI process, PPS, Medicare, Medicaid, and clinical computer systems. Participate in daily stand-up meetings to communicate changes in PPS timelines and deficiencies. This role requires Florida AHCA Clearinghouse background screening and at least one year of MDS experience.
