Job Summary

Assure the completion and accuracy of the Resident Assessment Instrument (RAI) process from the MDS through the implementation of the plan of care. Initiate and monitor RAI process tracking, discharge, and reentry while collecting data for each resident and interviewing staff to ensure compliance with standards of practice. Facilitate accurate determination of the Assessment Reference Date and provide an interdisciplinary schedule for all MDS assessments and care plans as required by OBRA and PPS. Assist the Director of Nursing with identifying significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect those changes. Coordinate scheduling for resident care planning conferences and communicate outcomes to staff, residents, and responsible parties. Correct and submit final MDS data to State and Federal agencies, maintaining validation records and addressing computer issues related to the MDS process.

Required Qualifications

  • Currently licensed as an LPN or RN in this state
  • CPR certified
  • At least 1 year of experience in MDS
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility
  • Excellent knowledge of the Federal Medicare PPS process, and Medicaid reimbursement
  • Knowledge of the OBRA regulations and Minimum Data Set
  • Knowledge of the care planning process
  • Demonstrated leadership ability
  • Excellent communication (oral and written) and public relations skills
  • Excellent organizational skills with an acute attention to detail
  • Computer literate
  • Florida AHCA Clearinghouse background screening
