MDS Coordinator

Job Summary
Assure completion and accuracy of the Resident Assessment Instrument (RAI) process from MDS through care plan implementation. Monitor RAI tracking, discharge/reentry, and validate assessment reference dates while collecting resident data and interviewing staff per MDS 3.0 standards. Coordinate interdisciplinary schedules for assessments and care plans, facilitate physician order updates for significant changes, and assign staff to the RAI process. Correct documentation deficiencies, submit final MDS data to State and Federal agencies, and attend Medicare or Utilization Review meetings as required.
Required Qualifications
- Currently licensed as an LPN or RN in this state
- CPR certified
- At least 1 year of experience in MDS
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of the Federal Medicare PPS process, and Medicaid reimbursement, as required
- Knowledge of the OBRA regulations and Minimum Data Set
- Knowledge of the care planning process
- Demonstrated leadership ability
- Excellent communication (oral and written) and public relations skills
- Excellent organizational skills with an acute attention to detail
- Computer literate
- Florida AHCA Clearinghouse background screening
