Job Summary
Coordinate, oversee, and ensure timely completion of MDS assessments in a Skilled Nursing Facility (SNF), ensuring CMS/MD regulatory compliance and accurate PDPM-relevant documentation. Lead interdisciplinary care plan meetings, monitor Case Mix Index (CMI) and PDPM reimbursement accuracy, track and submit Quality Measures (QMs), provide education on documentation and regulatory updates to nursing staff, and support survey readiness and audit responses. Strong emphasis on ensuring compliant coding, chart audits, and regulatory adherence to OBRA, Medicare PPS, and related requirements; rely on MDS 3.0 knowledge and EMR systems to maintain high-quality documentation and financial integrity.
Required Qualifications
- Active RN license in the state of New Mexico or licensed in any US state and eligible for New Mexico licensure by reciprocity
- Minimum 2-3 years of SNF clinical experience
- Strong knowledge of MDS 3.0 and PDPM
- Excellent organizational and communication skills
- Experience with electronic medical records (EMR) systems
