MDS Coordinator - RN

Job Summary
Oversee and coordinate MDS assessments for residents, collaborate with interdisciplinary teams to develop and update individualized care plans, ensure compliance with CMS/state/federal long-term care regulations, monitor clinical documentation for accurate reimbursement and quality measures, analyze resident data to support clinical decisions, serve as a resource on MDS processes, participate in care conferences, support audit readiness, enforce policies, and communicate care needs with residents and families. Requires an active RN license in Iowa, CPR certification, and knowledge of long-term care regulations; role includes on-call rotation with leadership team. This position is in Sioux City, IA and is an in-person, full-time opportunity with Care Initiatives.
Required Qualifications
- Valid RN license in good standing (Iowa) or ability to activate
- Current CPR Certification
- Knowledge of federal and state long-term care regulations and laws
- Strong commitment to empathy, respect, patience, and discretion
- Ability to collaborate with interdisciplinary teams
- Ability to communicate effectively with residents and families
- Document care accurately to meet regulatory and facility standards
