MDS Coordinator RN

Job Summary
Coordinate the completion of each resident's MDS in compliance with State and Federal requirements, maintaining supporting clinical record documentation. Facilitate the care planning process with interdisciplinary team members and ensure timely MDS completion. Assign ICD-10 diagnosis codes, monitor resident conditions, and review hospital records to accurately complete MDS scoring. Participate in weekly interdisciplinary care conferences, Medicare meetings, and Triple Check processes to review claims prior to billing. Educate staff on accurate documentation practices and lead Quality Assurance and Performance Improvement initiatives. Stay updated on current regulations related to long-term care reimbursement and MDS processes.
Required Qualifications
- Current CPR Certification
- Current RN license to practice in the State as an RN
- Ability to read, write, speak and understand English
- Working knowledge of the MDS assessment cycle and assessment information necessary for billing Medicare, Medicaid, HMO, and VA
- Experience using computer systems and software including proficiency in Microsoft Office Word and an email system
- Strong oral and written communication skills
