MDS Coordinator/RN

Coordinate the Medicare/MDS resident assessment process and notify the Interdisciplinary Team for MDS assessment completion in accordance with State and Federal regulations. Ensure the Interdisciplinary Team completes assessments timely while coordinating the development, implementation, and evaluation of the plan of care. Administer treatments, medications, or nursing interventions as indicated by the resident plan of care or physician orders, while providing care according to infection control standards and safety policies. Initiate emergency measures as needed per center policy. Requires current California RN license, two years of MDS Coordinator experience in a Skilled Nursing Facility, and a background check with no history of abuse.
