Job Summary
Coordinate, schedule, and complete all required MDS assessments (OBRA and PPS) accurately and in strict compliance with federal and state timelines. Facilitate interdisciplinary care plan meetings to establish individualized resident care plans. Monitor and optimize Patient-Driven Payment Model (PDPM) coding, ICD-10 diagnosis coding, and documentation to ensure accurate clinical reimbursement. Participate in weekly Triple Check meetings prior to billing submission and perform routine audits of clinical documentation. Educate floor nurses and interdisciplinary team members on documentation standards, MDS coding requirements, and care plan compliance. This full-time role is based in the Norwood Park area of Chicago, IL, at Norwood Crossing, a non-profit facility serving older adults.
Required Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Illinois
- Minimum 2+ years of experience as an MDS Coordinator in a Skilled Nursing Facility (SNF) or long-term care setting
- Deep understanding of CMS RAI guidelines, Medicare Part A/B, Medicaid, PDPM, and IDPH long-term care regulations
Desired Qualifications
- RAC-CT (Resident Assessment Coordinator-Certified) preferred
- Proficient with PointClickCare (PCC) or similar Electronic Health Record (EHR) systems
