Job Summary
Coordinate and document accurate resident assessments by interviewing patients and family members to gather clinical information. Maintain the MDS assessment calendar, monitor completion deadlines, and collaborate with interdisciplinary teams to finalize care plans and reviews. Ensure documentation is complete and submitted within required timeframes while supporting quality outcomes and reimbursement processes through MDS and PDPM adherence. Participate in on-call responsibilities as needed. This role supports resident outcomes and quality care within NHC HealthCare Anniston's interdisciplinary team.
Required Qualifications
- Current and active Registered Nurse license
- Strong organizational and time-management skills
- Excellent written and verbal communication skills
- Ability to collaborate effectively with an interdisciplinary team
- Compassionate, dependable, and detail-oriented
Desired Qualifications
- Previous nursing experience
- MDS and PDPM experience
- RAC-CT, RAC, or CMAC certification
