MDS Coordinator RN/LPN

Job Summary
Coordinate and oversee the MDS process and nursing assessments in the facility. Support the Director of Nursing in planning, organizing, developing, and directing daily Nursing Department operations to ensure compliance with federal/state/local standards. Ensure MDS 3.0 certification is maintained, complete resident assessments, coordinate nursing services with other resident services, participate in surveys and committees, maintain accurate documentation in resident records, and support training and quality initiatives. Must hold a valid RN or LPN license and have MDS 3.0 certification with related PDPM/ROP knowledge; expected to contribute to care planning, reporting, audits, and ongoing education for licensed nursing staff.
Required Qualifications
- MDS 3.0 Certification REQUIRED
- Valid RN/LPN license
- 2 years of experience or more with expertise with PDPM Patient Driven Payment Model and ROP Requirements of Participation
- Must be capable of performing essential nursing functions
- Ability to participate in facility surveys and OSHA/CDC in-service programs
