Job Summary
Complete timely and accurate MDS assessments in accordance with federal and state regulations, conducting resident interviews to gather comprehensive assessment data. Collaborate with interdisciplinary team members to develop individualized resident care plans, monitor implementation, and assist with care plan meetings and updates. Maintain documentation in compliance with regulatory and facility standards while staying current with MDS guidelines and clinical reimbursement processes. This full-time day shift role at Rio Grande Rehabilitation and Healthcare Center supports quality outcomes for residents in our skilled nursing facility.
Required Qualifications
- Current LPN or RN license in the State of Colorado or a compact state
- Knowledge of Medicare and Medicaid regulations related to MDS
- Strong attention to detail
- Excellent documentation skills
- Effective communication and collaboration with clinical and administrative teams
- Compassionate, resident-centered approach to long-term care
