Job Summary
Coordinate and conduct Minimum Data Set (MDS) assessments, care plans, and care area assessments for patients. Gather and document patient information, including medical, social, and functional data, while ensuring accurate and timely submission of all assessments. Maintain up-to-date records, identify discrepancies in patient data, and collaborate with physicians, nurses, and therapists to support compliance with federal and state regulations. Participate in quality improvement initiatives and educational programs. This role requires a high school diploma, 0-2 years of healthcare experience, and proficiency in electronic medical records and MDS software. Work Monday through Friday at our 175-bed facility in Rockville, Maryland.
Required Qualifications
- High school diploma or equivalent
- 0-2 years of experience in a healthcare setting
- Strong organizational and communication skills
- Ability to work effectively in a fast-paced environment and prioritize tasks
- Proficiency in electronic medical records and MDS software
- Familiarity with federal and state regulations regarding MDS assessments and care plans
