Job Summary
Complete Patient Assessment Process items on the MDS and accompanying Care Area Assessments (CAAs). Assist in developing individualized patient care plans and identify patient needs as required by Federal and State regulations. Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process, including computer encoding and electronic transmission of the MDS. Identify changes in patient condition with the interdisciplinary team and coordinate scheduling for indicated assessments. Assist with Activities of Daily Living training for new hires and ongoing continuing education for existing staff. Attend educational programs to maintain knowledge of current RAI and Prospective Payment System regulations. Full-time Monday through Friday schedule.
Required Qualifications
- Must be a Registered Nurse currently licensed by the state and in good standing
- Previous experience in a hospital, long-term facility, or other healthcare related facility
- Prior experience completing MDSs and Care Plans
- Knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care
