Job Summary
Conduct daily patient assessments by completing MDS forms and accompanying Care Area Assessments in alignment with Federal and State regulations. Coordinate scheduling for interdisciplinary Resident Assessment Instrument (RAI) processes, including computer encoding and electronic transmission. Assist in developing individualized patient care plans while identifying projected outcomes and changes in patient conditions. Provide Activities of Daily Living training for new hires and support ongoing continuing education for existing staff. Maintain current knowledge of RAI and Prospective Payment System regulations through educational programs. Requires RN licensure and prior experience with MDSs and care plans in a long-term care setting.
Required Qualifications
- Must be a Registered Nurse currently licensed by the state and in good standing
- Previous experience in a hospital, long-term facility, or other healthcare related facility
- Prior experience completing MDSs and Care Plans
- Knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care
