Job Summary
MDS Coordinator role responsible for completing and coordinating accurate MDS assessments, collaborating with interdisciplinary teams to develop individualized care plans, ensuring CMS/state regulatory compliance, validating clinical documentation for appropriate reimbursement and quality measures, analyzing resident data to support clinical decision-making, serving as a resource on MDS processes, participating in care conferences, supporting audit and survey readiness, enforcing policies, communicating with residents and families about care needs, maintaining infection control and safety protocols, and participating in an on-call rotation with leadership peers. Requires an active Iowa RN license, CPR certification, and knowledge of long-term care regulations; position emphasizes compassionate, patient-centered leadership within a skilled nursing environment.
Required Qualifications
- Valid RN license in good standing (Iowa) or ability to activate license
- Current CPR Certification
- Knowledge of federal and state long-term care regulations and laws
- Desire to learn and grow as part of a quality-focused clinical team
- Strong commitment to empathy, respect, patience, and discretion in resident care
