Job Summary
Complete and coordinate accurate, timely MDS assessments to evaluate resident clinical and functional needs. Collaborate with interdisciplinary teams to develop and update individualized care plans while ensuring compliance with CMS, state, and federal long-term care regulations. Monitor and validate clinical documentation to support accurate reimbursement and quality measures, analyzing resident data and trends to support care quality and clinical decision-making. Serve as a resource to nursing and clinical staff on MDS processes and documentation standards, participating in care conferences and supporting audit readiness. Enforce policies, guide the team, and communicate effectively with residents and families regarding care needs. Follow infection control, safety, and compliance protocols at all times and participate in an on-call rotation with other members of the leadership team.
Required Qualifications
- Valid RN license in good standing
- License must be valid within the state of Iowa, or ability to activate
- Current, valid CPR Certification
- Knowledge of federal and state long-term care regulations and laws
