MDS Coordinator - RN (85745)

Ensure accurate, timely, and complete MDS assessments for Admission, PPS, Quarterly, Annual, and Significant Change events in compliance with RAI guidelines. Complete Medicare and Medicaid case-mix documentation to support reimbursement and avoid default days. Work with the Director of Rehab and interdisciplinary team to select optimal ARDs and support effective care planning. Track Skilled and Managed Care residents for eligibility, benefit utilization, and timely documentation. Gather information for Managed Care reviews, physician certifications, and re-certifications. Conduct regular audits of MDS coding, outcomes, and validation reports to maintain CMS policy compliance. Submit assessments electronically, review error reports, and address late or rejected submissions. Provide training and guidance to staff on the RAI process, documentation requirements, and regulatory updates. Serve as a clinical reimbursement resource for nursing, therapy, business office, and administration. Actively participate in Case Management, Triple Check, and Level of Care meetings. Maintain communication regarding case mix, reimbursement impact, and clinical documentation quality. Support resident-centered care planning by ensuring resident preferences and clinical changes are accurately reflected.
