MDS Coordinator (Registered Nurse/RN)

Job Summary
RN MDS Coordinator coordinates and assists with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations and Life Care standards; requires an active RN license, CRN C, CPR certification, and at least two years of nursing experience (geriatric experience preferred); responsibilities include staff education, compliance review, coordination of CMS QIES submissions, and participation in PPS/Committee meetings while maintaining patient safety and confidentiality.
Required Qualifications
- Associate’s or bachelor’s degree in nursing from an accredited college or university
- Active Registered Nurse (RN) license in good standing
- Two (2) years’ nursing experience (geriatric nursing preferred)
- CRN C Certification (clinical compliance)
- CPR certification upon hire or during orientation (current during employment)
- Advanced knowledge in field of practice
- Ability to make independent decisions
- Knowledge of CMS RAI Manual Regulations and post-acute care regulations
- Ability to implement and interpret programs/policies
- Ability to perform competency-related tasks (patient rights, safety, sanitation)
- Maintain confidentiality
- Adhere to Code of Conduct and compliance training
- Coordinate and assist with MDS Assessments, CCAs, and Care Plans; review CMS reports; participate in meetings
- Demonstrate excellent customer service and teamwork
- Physical ability to sit/stand/bend/lift/move as required
- English language proficiency
