MDS Coordinator (Registered Nurse/RN)

Job Summary
Coordinate and assist with completion and submission of accurate, timely interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations. Report patient condition changes to the Director of Nursing, provide education to direct care associates on regulatory updates, and review Final Validation Reports before billing. Attend Daily PPS Meetings, Monthly Triple Checks, and other required team meetings while performing staff nurse functions as needed. Maintain confidentiality, adhere to Life Care's Code of Conduct, and ensure all assessments are accepted into the CMS QIES system. This role supports the University Park Care Center's 5-star CMS rating and recognition as a Best Nursing Home in Colorado.
Required Qualifications
- 2 years nursing experience
- Associate's or bachelor's degree in nursing from an accredited college or university
- Currently licensed/registered in applicable State
- Must maintain an active Registered Nurse (RN) license in good standing throughout employment
- Two (2) years' nursing experience
- CPR certification upon hire or obtain during orientation
- CPR certification must remain current during employment
- Advanced knowledge in field of practice
- Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
- Demonstrate dependable, regular attendance
- Concentrate and use reasoning skills and good judgment
- Communicate and function productively on an interdisciplinary team
- Read, write, speak, and understand the English language
- Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
