MDS Coordinator (Registered Nurse/RN)

Job Summary

Coordinate and assist with completion and submission of accurate, timely interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations. Report patient condition changes to the Director of Nursing, educate direct care associates on regulatory updates, and review final validation reports before billing. Attend daily PPS meetings, monthly triple checks, and other interdisciplinary team meetings as needed. Perform staff nurse functions, assist in patient evacuations, and maintain confidentiality while adhering to company policies and the Code of Conduct. This full-time role includes an on-call rotation once every 9 days at Life Care Center of Longmont, a US News and World Report Best Nursing Home recognized for quality measures and longevity.

Required Qualifications

  • Associate's or bachelor's degree in nursing from an accredited college or university
  • Currently licensed/registered in applicable State
  • Must maintain an active Registered Nurse (RN) license in good standing throughout employment
  • Two (2) years' nursing experience
  • CRN C Certification (clinical compliance)
  • CPR certification upon hire or obtain during orientation
  • CPR certification must remain current during employment
  • Advanced knowledge in field of practice
  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
  • Read, write, speak, and understand the English language

Desired Qualifications

  • Geriatric nursing experience preferred
