MDS Coordinator (Registered Nurse/RN)

Job Summary
RN MDS Coordinator coordinates and assists with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Life Care standards. Responsibilities include reporting changes in a patient’s condition identified by the MDS Assessment to the DON; providing education to direct care staff regarding CMS RAI Manual updates; reviewing the Interdisciplinary Comprehensive Care Plan; attesting that assessments are completed and accepted into the CMS QIES system prior to billing; reviewing CMS Reports to identify late submissions and developing systems to prevent reoccurrence; attending PPS and other meetings; performing staff nurse functions as required; maintaining professionalism, confidentiality, and compliance with policies; and supporting a positive patient-care environment. Essential skills include advanced knowledge in the field, independent decision-making, knowledge of post-acute care regulations, effective communication within an interdisciplinary team, and physical ability to perform typical nursing tasks. “An Equal Opportunity Employer.” Location: Longmont, CO.
Required Qualifications
- Associate’s or bachelor’s degree in nursing from an accredited college or university
- Currently licensed/registered in applicable State with active RN license in good standing
- Two (2) years’ nursing experience; geriatric nursing experience preferred
- CRN C Certification (clinical compliance)
- CPR certification upon hire or obtain during orientation (must remain current)
