Job Summary
RN MDS Coordinator travels to skilled nursing facilities to complete and submit MDS assessments, CCAs, and care plans per CMS RAI Manual regulations. Responsible for coordinating and assisting with accurate, timely documentation; reviewing CMS reports and attestation of completed assessments; providing education on CMS updates to direct care staff; participating in PPS meetings and other required reviews; performing staff-nurse duties as needed; maintaining confidentiality and professional conduct; and ensuring compliance with regulations while coordinating with an interdisciplinary team. The role requires travel between facilities, strong organizational and communication skills, and adherence to Life Care standards and company policies.
Required Qualifications
- Associate’s or bachelor’s degree in nursing
- Active RN license in good standing
- Two years nursing experience (geriatric nursing preferred)
- CRN C Certification (clinical compliance)
- CPR certification upon hire or during orientation (current during employment)
- MDS Assessments coordination and submission
- CMS RAI Manual Regulations knowledge
- Interdisciplinary collaboration
- Ability to travel between facilities
Desired Qualifications
