MDS Coordinator

Oversee and facilitate resident assessments in accordance with Federal and State regulations, establishing accurate and timely schedules. Transmit assessments to the State of Connecticut, Department of Health and coordinate care plans to ensure effective delivery. Initiate Medicare coverage for newly qualified residents, manage denial letters, and determine necessary reimbursement documents. Coordinate and complete Minimum Data Sets, conduct regular resident interviews, and facilitate interdisciplinary care conferences. Educate nursing staff on the MDS process and ensure current updates are implemented. Requires a licensed Massachusetts RN or LPN with at least one year of MDS experience.
