Job Summary
MDS Coordinator coordinates resident assessments and care plans (MDS) and oversees PPS/OBRA assessments and related RAPs/Triggers, transmits completed MDS weekly, maintains 15 months of MDS records for surveys, monitors care plan conferences, and participates in on-site surveys. The role also involves performing nursing functions when required and performing administrative duties; requires RN/LPN licensure and managerial experience, with proficiency in office software, strong communication and customer service, and ability to work in an interdisciplinary team.
Required Qualifications
- Registered Nurse with a current, unencumbered license in the state of operation
- At least two (2) years of managerial experiences in a hospital or nursing care preferred
- Able to demonstrate the knowledge and skills necessary to prove care appropriate to the age-related needs of the residents served
- Schedule flexibility, with ability to be rotating managerial on-call
- Able to problem solve independently when warranted, demonstrates knowledge of medical practices and procedures as well as laws, regulations, and nursing care communities
- Basic knowledge with office software applications including Word, Outlook and Excel
- Must be able to read, write and speak the English language
- Must possess excellent customer service skills
