MDS Coordinator

Lead MDS assessment coordination for resident care. Organize Medicare/MDS resident assessment process and ensure timely completion by the Interdisciplinary Team (IDT). Coordinate the development, implementation, and evaluation of care plans; administer prescribed treatments, medications, and nursing interventions; provide necessary nursing care with infection control standards. Follow safety protocols, initiate emergency measures per center policy and nursing standards; facilitate communication for quality resident care; maintain required documentation per Federal/state regulations and company policies; participate in meetings and training; uphold professional standards in addressing consumer inquiries.
