Job Summary
MDS Coordinator role in a management position located in Kaplan, LA. Responsibilities include completing MDSs and Care Plans in a skilled nursing facility, with proficiency in RAI/PPS processes, and strong documentation, communication, and organizational skills. On-site role with a Monday-Friday schedule, 8-hour shifts, and on-call requirements. RN or LVN/LPN license required, plus graduation from an accredited nursing program and 1+ years of skilled nursing/MDS experience. Starting hourly rate: RN $35-$40/hr or LPN $28-$33/hr, with $1.50 more per hour after 30 days following education completion.
Required Qualifications
- Current RN or LVN/LPN license in state of practice
- Graduate of an accredited school of nursing
- 1+ years of experience in skilled nursing facility with previous experience in completing MDSs and Care Plans as an MDS Coordinator
- Demonstrated proficiency in the RAI / PPS processes
- Strong attention to detail, documentation, and resident assessment skills
- Good communication, organizational, and computer skills
