MDS Coordinator

Job Summary
MDS Coordinator sought to perform accurate MDS assessments and care plans, initiate and update care plans, create monthly care plan calendars, monitor and audit clinical records for timeliness and accuracy, inform the DON of non-compliant documentation, protect resident and facility confidentiality, and participate in meetings/committees such as Medicare and QAPI. Requires RN or LPN license, MDS experience in a long-term care/skilled nursing setting, and 40-hour workweek; strong organization, communication, and computer skills are essential.
Required Qualifications
- Valid RN or LPN License
- Must have prior MDS Experience
- Long Term Care Experience Required
- Must be able to work 40 hours a week
- Must be highly organized, professional & motivated
- Should have solid computer skills
- Excellent communication skills
- Should be friendly and a team worker
