Job Summary
Complete accurate MDS assessments and care plans as assigned, initiating support activities and creating monthly care plan calendars. Maintain and update all clinical records while monitoring and auditing for accuracy and timeliness. Inform the Director of Nursing of persistent non-compliant documentation issues and protect resident and facility confidentiality. Requires prior MDS 3.0 experience, a valid New York RN license, and long-term care experience. Based at a 200-bed rehabilitation and skilled nursing facility in White Plains, NY.
