Job Summary
Coordinate and complete the MDS/RAI process for all residents in accordance with state, federal, and facility guidelines. Develop, update, and oversee resident-centered care plans while reviewing clinical documentation, nursing notes, and physician orders to support accurate assessment and ICD-10 coding. Lead care conferences, QAPI meetings, and staff education on documentation standards to monitor quality indicators and ensure compliance with CCH Corporation policies and survey readiness. This Full-Time MDS Coordinator role at Maple Grove Nursing & Rehabilitation in Cincinnati, OH requires an RN with strong assessment skills and is open to candidates with prior MDS experience.
Required Qualifications
- Current, valid Registered Nurse (RN) license in the State of Ohio
- Prior MDS Coordinator experience in long-term care
- Working knowledge of MDS, RAI process, PDPM/Medicare/Medicaid reimbursement, and care planning
- Strong clinical assessment, critical thinking, and organization skills
- Accuracy-focused with excellent attention to detail and follow-through
- Effective communicator and team player, comfortable collaborating with nursing, therapy, social services, and providers
- Proficient with electronic health records and computer-based documentation
Desired Qualifications
- Prior MDS Coordinator experience in long-term care strongly preferred
