Job Summary
Coordinate Medicare/MDS resident assessments and lead the Interdisciplinary Team (IDT) for MDS completion; coordinate development, implementation and evaluation of resident care plans; administer or coordinate nursing interventions and medications as ordered; ensure infection control and safety policy adherence; and initiate emergency measures under the direction of the Director of Nursing at French Park Care Center in Santa Ana, CA.
Required Qualifications
- Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license
- Experience with Medicare/MDS processes and resident assessment
- Ability to coordinate Interdisciplinary Team (IDT) for MDS completion
- Knowledge of State and Federal regulations governing MDS and nursing practice
- Ability to develop, implement and evaluate care plans
- Strong infection control and safety policy adherence
- Ability to administer or coordinate nursing interventions and medications as ordered by physician
- Supervision under Director of Nursing (DON)
- Excellent communication and coordination skills
