MDS Coordinator/LVN

Coordinate the Medicare/MDS resident assessment process and notify the Interdisciplinary Team for MDS assessment completion in accordance with State and Federal regulations. Ensure the team completes assessments timely while coordinating the development, implementation, and evaluation of resident plans of care. Administer treatments, medications, or nursing interventions as indicated by the plan of care or physician orders, adhering to infection control standards and safety policies. Initiate emergency measures according to center policy and nursing practice standards. Requires current California LVN license, two years of MDS Coordinator experience in a Skilled Nursing Facility, and a background check with no history of abuse.
