Job Summary
Conduct and complete Minimum Data Set (MDS) assessments to evaluate residents' physical, psychological, and functional status, including Care Area Assessments and triggers. Evaluate resident conditions and medical data to determine needs for special assessment activities or admission assessment amendments. Prepare and electronically transmit timely reports to national Medicare and Medicaid databases. Develop written plans of care identifying resident problems, needs, and goals. Provide information to residents and families regarding Medicare/Medicaid and financial assistance programs. Ensure MDS notes are informative and descriptive of services provided and resident responses. Assist with discharge plan care portions and implement committee recommendations for assessment and care plan functions.
Required Qualifications
- Must have and maintain a current, valid state LPN or RN license
- Current, valid CPR certification required
Desired Qualifications
- Three (3) to five (5) years' experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred
