MDS Coordinator (LPN or RN)

Job Summary
Conduct and coordinate Minimum Data Set (MDS) assessments and care planning while ensuring compliance with federal, state, and local regulations. Submit assessments to the State Repository per RAI Manual guidelines and collaborate with the Interdisciplinary Care Team to set review dates. Evaluate and update resident care plans based on health status changes or quarterly assessments, and monitor Quality Measures Reports to maintain high Five-Star Ratings. Educate nursing staff, residents, and families on personalized care plans and participate in facility surveys and inspections. Maintain strict confidentiality and uphold patient privacy standards. Requires an active LPN or RN with 2+ years of experience; offers competitive compensation, daily pay, and comprehensive benefits.
Required Qualifications
- Active, unencumbered Licensed Nurse (LPN/RN) in the state
- Nursing Degree/Diploma from an accredited school, college, or university
Desired Qualifications
- 2+ years of experience in a hospital, skilled nursing, or healthcare facility preferred
