Job Summary
MDS Coordinator LPN position focused on coordinating the resident assessment process in accordance with Federal and State regulations and company policy. Responsibilities include coordinating and completing the Resident Assessment Instrument (RAI) with the Interdisciplinary Team, notifying team members of MDS schedules and changes, ensuring timely transmission and compliance with Federal regulations, developing and monitoring plans of care based on MDS and CAA outcomes, leading Interdisciplinary Care Plan meetings, and providing ongoing education on the RAI process to staff and appropriate stakeholders. Requires current LPN licensure, CPR certification, 3+ years of clinical experience (long-term care preferred), strong knowledge of regulatory requirements, proficiency with Microsoft Office, excellent communication and organizational skills, and the ability to work with minimal supervision and handle multiple projects. Location in Suffolk, VA; role involves onsite responsibilities with no remote work indicated.
Required Qualifications
- Licensed Practical Nurse with required current state licensure
- Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred
- Must have a current/active CPR certification
- Knowledgeable of nursing and medical practices and procedures
- Self-motivated to learn and master the RAI Process
- Effective verbal and written English communication skills
- Demonstrated basic to intermediate skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation
- Highest level of professionalism with the ability to maintain confidentiality
- Ability to communicate at all levels of organization and work well within a team environment in support of company objectives
- Customer service oriented with the ability to work well under pressure
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity
- Strong analytical and problem solving skills
- Ability to work with minimal supervision, take initiative and make independent decisions
- Ability to deal with new tasks without the benefit of written procedures
- Approachable, flexible and adaptable to change
- Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors
