MDS Coordinator (LPN)

Job Summary
MDS Coordinator role responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring timely and accurate resident assessments across Medicare/Medicaid/OBRA requirements, and collaborating with the interdisciplinary team to support care plans. Requires LPN licensure, CPR certification, and at least two years of healthcare experience. Key duties include MDS assessment timing and accuracy, PPS/RUG/PDPM alignment, Medicare/Managed Care eligibility, data integrity, regulatory compliance, and ongoing trainings (Relias). Strong communication, organizational, and problem-solving skills are essential to report and support services and ensure appropriate reimbursement; supervision of RAI processes and training of new associates are also part of the role.
Required Qualifications
- Graduate of an accredited LPN school of nursing
- Valid LPN license in the state employed
- Valid CPR certification
- Minimum of two years health care experience
- Experience with MDS completion, reimbursement, clinical resource utilization and/or case management is highly desirable
