Job Summary
Coordinate the Resident Assessment Instrument (RAI) process, including completing accurate Minimum Data Sets (MDS) and developing the Interdisciplinary Plan of Care. Participate in daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meetings and establish positive relationships with the healthcare team while functioning as part of the nursing management unit. Requires a current, unencumbered LPN license with at least one year of direct hands-on nursing and MDS/RAP experience. The role involves utilizing local and consultant resources to navigate state, federal, and regulatory requirements. Full-time team members receive a competitive wage range of $36.00 - $42.00 per hour alongside a comprehensive benefit package including medical, dental, vision, and tuition reimbursement.
