Job Summary
MDS Coordinator LPN is responsible for conducting resident assessments, developing individualized care plans with the interdisciplinary team, maintaining accurate documentation of assessments and progress, implementing strategies to improve resident outcomes and reduce readmissions, participating in quality improvement initiatives, and handling admissions, discharges, and transfers while adhering to infection control practices.
Required Qualifications
- Licensed Practical Nurse (LPN) with current licensure
- 1+ years of experience in MDS
- Familiarity with MDS regulations and guidelines
- Basic knowledge of electronic medical records and other software applications
