MDS Coordinator LPN

Provide leadership to the interdisciplinary team for managing MDS, clinical reimbursement, care plan processes, and utilization management in accordance with federal, state, and local regulations. Ensure MDS/RAI deadlines and accuracy requirements are met while optimizing coding to capture patient acuity for accurate reimbursement. Assist center leadership with quality improvement efforts, staff education, and regulatory compliance regarding the RAI process and quality reporting. Maintain a positive, collaborative team environment while demonstrating strong computer skills and the ability to work independently under pressure. Requires current RN license in good standing in VA, significant MDS 3.0 experience, and RAC-CT certification within one year.
